  2. To set up your first appointment we will need your reason for needing Physical Therapy (diagnosis and date of surgery if applicable), the referring doctor's name, your insurance information, date of birth, phone number and email address.
  3. Next you will receive an email from us with our intake paperwork packet. Mailing or pick up is also available. All forms MUST be completed prior to your initial evaluation.
